Responsibilities
Duties will include:
- Provide support to the users of the service to assist them to maintain their mental and physical well-being in a person-centred approach
- Develop and work with person centred, outcome-based care plans which reflect the needs of individuals
- Support service user who are in crisis and struggling with their mental well being
- Promote social stimulation and community integration with users of the service
- Enable users of the service to develop daily living tasks with a view to stepping down to more independent living as much as possible. For example, cooking, shopping, personal care, managing finances, personal safety
- Work with professionals and refer to relevant professionals when required. For example, attending meetings, reviews
- Maintain documentation required by Proudfoot Care Group and Care Quality Commission
- Follow Policies and Procedures as directed
- Promote the business in a positive and professional manner
- Undertake training as required by Management
- Have a working knowledge and understanding of relevant legislation required for the protection of the users of the service

Training Provided
Level 2 Adult Care Worker apprenticeship standard, which includes:
- Induction
- Care certificate
- All other training required which is client led
- Level 1/2 Functional Skills in maths and English (if required)
- End-Point Assessment (EPA)
